Care Services is a client driven support program dedicated to providing a broad range of quality and consistent support services to UCAN’s diverse client base. UCAN’s Direct Services include:
Case Management Program works one-on-one with people living with HIV/AIDS to ensure access to primary medical care and social services in order to empower people to become self-sufficient.
Client Centered Advocacy with one-on-one meetings to support clients in determining what services they may need. Clients receive individual counseling from the Client Services Manager about services available to meet the legal, financial, medical and housing needs of people living with HIV/AIDS.
Prevention With Positives provides one-on-one support to develop an individual service plan in maintaining health and safety.
Buddy/Companion Program provides one-on-one emotional and social support from trained volunteers. Buddies visit clients on a weekly basis, and can also provide limited practical assistance such as transportation, light housekeeping and shopping.
Drop-in Center offers a warm and safe place for clients to rest, eat light meals, watch videos, make telephone calls, network with other clients and access the Internet and read published information about HIV/AIDS.
Direct Service Fund provides emergency financial assistance for expenses related to food, housing, utilities, transportation and medical.
Support Group is a safe place for clients to share stories, information, and concerns; to decrease isolation; and to give and receive emotional support. Meeting weekly, it is facilitated by a local mental health counselor experienced in issues related to HIV/AIDS.
Basic Needs Closet is stocked with donated toiletries and food from community partners to deliver ongoing support.
